Basic- Pay per visit

$300 for the initial 90 min evaluation for any combination of the services listed on the website including primary care services if needed.

$180 for 45 minute follow up visits.

Concierge Membership Services with Psychiatric & Pain Management Services.

At Mountain Air Wellness, we offer a personalized concierge medical membership program designed to enhance access, communication, convenience, and continuity of care.

The annual concierge membership fee is $5,500 and covers non-covered administrative and enhanced care services not reimbursed by insurance plans, including Medicare.

Insurance Billing

For patients with commercial insurance or Medicare:

  • Medically necessary covered services will continue to be billed to your insurance carrier in accordance with applicable laws and payer requirements.

  • The concierge membership fee does not replace insurance billing for covered medical services.

  • Patients remain responsible for maintaining active health insurance coverage.

  • As part of the concierge program, copays, coinsurance, and routine out-of-pocket costs for covered services provided by the practice may be waived or absorbed under the membership agreement, where legally permitted.

Medicare Patients

The concierge membership fee covers only non-covered services and enhanced access amenities that Medicare does not reimburse.

All Medicare-covered medical services will continue to be billed to Medicare unless the provider has formally opted out of the Medicare program.

The membership fee is not a substitute for Medicare coverage and does not cover any professional medical services.

Medicaid / Health First Colorado

Due to Colorado Medicaid regulations and compliance requirements:

  • Patients enrolled in Colorado Medicaid (Health First Colorado) are not eligible to participate in the concierge membership program.

  • Medicaid-covered services must be billed directly through the Medicaid program.

  • The practice cannot accept concierge fees or direct cash payment from Medicaid beneficiaries for services covered under Medicaid.

Important Notice

The concierge membership fee is for enhanced access, administrative conveniences, and non-covered services only. It is not insurance and does not replace health insurance coverage.

Patients are encouraged to maintain comprehensive medical insurance coverage, including Medicare or a commercial plan, as applicable.

Pain Managment Services 

Comprehensive pain management services, including medication management, referrals for procedures, and psychedelic-assisted therapy for pain management. Follow-up appointments can be done via tele-health. In office initial evaluation and annual in-office follow-up required annually to complete annual urine drug screen as recommended by the state of Colorado.

  • Safe and Supportive Environment: Our office is designed to provide a calming and supportive atmosphere, ensuring your comfort and safety with handicap access and empathic care. Please notify the office if you need handicap accessibility so we can show you the elevator on your first visit.

  • Follow-Up: A variety of follow-up options for ongoing controlled substance prescribing. Patients must sign a pain contract if controlled substances such as opioids are used and complete annual urine drug screens. Patients must also agree to provider monitoring of the pharmacy database to ensure safe prescribing.

This may include dietary recommendations, exercise plans, behavioral therapy, and, if appropriate, medication management, including Semaglutide (Ozempic or Wegovy). Our pharmacy can compound this medication, making it much more affordable for you. Typically, semaglutide is the most affordable option, with trezepitide usually about twice as expensive, even when compounded. Pricing is listed below. Metformin has been shown to be a good option to prevent rebound weight gain following this treatment if patients want to stop the injection of Semaglutide. These are all options we can discuss as we review your individual goals.

Semaglutide (GLP-1)/ Methylcobalamin (B12) Weight Management Program

Initial Evaluation – $200
A comprehensive 60-minute telehealth or in-person visit focused exclusively on assessing eligibility for GLP-1 therapy.

Semaglutide 2.5 mg/mL /Methylcobalamin (B12) 1mg/mL Injection Solution– 5 mL Vial ($350)

Provides enough medication for just under 4 months of treatment.
A second vial is typically ordered at the start of month 4.
Approximate cost: $94/month for the first 4 months, including shipping to the office or your home.

Dosing Schedule (First 5 mL Vial):

  • Weeks 1–4: Inject 10 units (0.1 mL / 0.25 mg) subcutaneously once weekly

  • Weeks 5–8: Inject 20 units (0.2 mL / 0.5 mg) once weekly

  • Weeks 9–12: Inject 40 units (0.4 mL / 1 mg) once weekly

  • Weeks 13–15: Inject 70 units (0.7 mL / 1.75 mg) once weekly

This completes the first 5 mL vial.

3-Month Follow-Up – $130

A 30-minute evaluation to review progress, adjust dosing, and ensure ongoing safety.

Mountain Air Wellness is located in the Elevated Wellness Collective in Avon, Colorado, just off of I-70, exit 167 on the north side of the interstate, behind and above the Exxon Mobil Gas Station. To get to our office, drive past the Exxon and take the first left before the fire department and Buck Creek Medical Center. Our driveway is the first one on the right behind the Exxon.
